The
Siniloan Water District intends to procure the “Supply and Delivery of Dry Chlorine” with Approved Budget for the
Contract of Five Hundred Eighty Thousand
Pesos ( PhP 580,000.00) through Small Value Procurement pursuant to section
34 of the Implementing Rules and Regulation (IRR) of Republic Act (RA) No.
12009.
Please
submit your duly signed Quotation Form including the Terms and conditions
(Annex A) together with the following documents on or before 1:00
PM of 2 June 2025 and
will be opened on the same day at 1:30 PM at SIWADI Building Brgy. P. Burgos Siniloan, Laguna.
a. PhilGEPS Registration Certificate
b. Mayor’s/Business Permit
c. Notarized Omnibus Sworn and if applicable, Original Notarized Secretary’s Certificate in case of a corporation, partnership, or cooperative; or Original Special Power of Attorney of all members of the joint venture giving full power and authority to its officer to sign the OSS and do acts to represent the Bidder. d. Supplier’s latest Income Tax
e. Copy of DTI or SEC registration Certificate
f. Audited Financial Statement supported by an independent CPA’s certificate, stamped ”receive” by the BIR or its duly accredited an authorized institutions, for preceding calendar year, which should not be earlier than two (2) years from the date of bid submission shall be attached upon submission of the quotation
g. Authority of the representative/signatory signing the price quotation.
